Day, who moved into the world’s top five on the back of the win, made crucial putts at the 16th and 17th holes on his way to joining a four man play-off at 9-under-par.
Harris English and Scott Stallings fell out of sudden death with pars at the first extra hole, Torrey Pines’ par-5 18th, while Day and JB Holmes headed to the par-3 16th to decide the title.
Day’s par was enough after Holmes missed the green and couldn’t get up and down.
Starting the week as one of the tournament favourites after his runner-up finish last year Day was around the lead all week.
The win moved him to fourth in the world rankings.
Day’s final round 70 was the best of the four Australians who made the cut.
Next best Marc Leishman, also a runner-up here last year, started the final day inside the top 15 but fell away with a 4-over-par 76 to drop to T27.
John Senden (T49) and Cameron Percy (T53) were the only other Australians to make the weekend.
Robert Allenby, Aaron Baddeley, Cameron Smith, Greg Chalmers, Stuart Appleby and Jarrod Lyle all missed the halfway cut.
